In Pinellas County, we are fortunate to have several programs that can help you in your home. Once program that’s particularly unique is called PACE: Program for the All-Inclusive Care for the Elderly. It’s an excellent program designed to keep you in your home.
They come to you; they transport you to their facility where your physician and your pharmacist and your nurse care manager are. It brings continuity to the care that you get, where all your professionals are on the same page or in the same room. They know what is going on with you, and they help you with transportation to and from your home.
